Integrated High-Risk Pregnancy Tracking and Management
The INTEGRATED HIGH RISK PREGNANCY TRACKING AND MANAGEMENT (IHRPTM) programme focuses on training and supporting Auxiliary Nurse Midwives (ANMs), Medical Officers (MOs) and Specialist Doctors on high risk pregnancy tracking, management and referral.
IHRPTM uses a comprehensive, multi-step, systemic approach for
- improved identification,
- tracking and
- end-to-end management of high-risk pregnancies.
This approach enables a sustained reduction in irrational and delayed/complicated referrals and an improved continuum of care for maternal health, towards achieving the goal of Ending Preventable Maternal Mortality (EPMM).

IHRPTM, also called Amma Kosam in Telangana & Andhra Pradesh, has now expanded to Uttar Pradesh with the initial pilot phase commencing in two districts of Shravasti (aspirational district) & Sambhal, in the State. The programme aims to scale up to two more states by 2026-27.
ARMMAN has developed 35 comprehensive, evidence-based, colour-coded, simplified algorithmic guidelines for end-to-end management of all antenatal high-risk factors for health workers at every level of the health system.
Of these, protocols for 20 high-risk conditions were adapted for Telangana and 18 high-risk conditions were adapted by Andhra Pradesh, based on the respective states’ mortality data and accepted as policy by the state.

The programme is implemented in partnership with UNICEF and the State Governments of Telangana, Andhra Pradesh & Uttar Pradesh.

The ANMs, MOs, and Specialists trained through this programme will also be supported by a learning app with rich multimedia-based approaches and by a tablet-encoded decision supported tool linked to the live mother and child database, enabling end-to-end tracking of high-risk conditions and creation of integrated dashboards at every level.

Innovations using AI
We have developed a Large Language Model (LLM) which is integrated into the existing learning and support application to improve the training of ANMs so they can better manage high-risk pregnancies.
We have developed a chatbot to provide personalised answers to the queries raised by the ANMs, depending on their training level. This has reduced the burden on Medical Officers (who are currently responding to each query), improved response time, and enabled hand-holding at scale.
